Concierge Health Network is a provider network with real tools behind it: scheduling and charting on Jane, a working relationship with My Concierge Pharmacy, and clinical programs for patients who need more than a once-a-year appointment. You practice medicine. We help with the pieces that usually fall apart after the visit.
Most of us are fine in the exam room. The hard part is everything around it: referrals that never land, prescriptions that never get filled, and patients who disappear until something goes wrong. A network only helps if it changes that day-to-day reality.
CHN is the provider layer. Around it are three practical pieces. You do not have to build all of this yourself.

My Concierge Pharmacy handles dispensing and follow-through after the visit, so medication plans are less likely to stall at the counter.
For patients who need monitoring or structured follow-up between office visits, we use clinical program partners (including remote patient monitoring where appropriate).
You remain the clinician. CHN is how patients find you, how visits are scheduled, and how pharmacy and programs stay connected to that care.
